Once you decide to donate your brain, fill out the forms and send them in to the Brain Bank of your choice.
It is necessary to discuss your wishes with your family and next of kin. All caregivers need to be aware of a loved one's plans at the time of death, ie, nursing homes, funeral homes, pathologists. It is suggested that these plans should be discussed and made in advance to be sure that no issues arise, such as availability of a pathologist, and so that the family has no extra unnecessary concerns at an already stressful time.
Inform your medical physicians and institutions that you want your medical records to be sent to the Brain Bank. Contact the harvester / pathologist and notify the funeral home of your wishes.
Confirm with the pathologist and funeral home as death is imminent to confirm plans.
On death contact funeral home and harvester and institution informing them about the donation.
After the brain is shipped to the Brain Bank get confirmation that they received the tissue.
Ensure that the medical records are sent to the Brain Bank.
Request a postmortem and autopsy report of the brain.
Once the family received the autopsy report distribute it to all the physicians and medical personnel who your loved one had seen so that they have a clinical diagnosis - which is the whole point.
