Make copies of this form and send to the instituition and keep copy with your advanced medical directives.
Give permission to have medical records released tp omstotitopm wjere brain is donated to.
Select funeral home and make sure harvester has access to remove the brin.
Set up with harvester
Make arrangements for harvesting.
Note brain needs to be removed from the body within 24 hours and preferably 12hours. The sooner the better. This means that the funeral home or hospital must cooperate with the harvester.
